Really nice form that is super easy to use and configure. used this one a few times now and would highly recommend if someone is looking for an out of the box easy solution.

Really nice, easy to use form builder. Would love a little more advanced version that provides a few form layout options and would allow changes to the order of form fields. But, this is great for what it was intended, simple one-column forms with the options to add
auto-responders, admin alert, etc....

This works as advertised. You can't beat it for the price. I had to ask for support for what turned out to be a systemic problem on my site and the response was timely and proficient.

Great module, but it does not have the ability to order the fields and their is a limit of 3 custom text fields and 2 custom drop downs. Seems like a better design would be to allow an arbitrary number. I like the module though.

Simple and cheap. Very little customization though. Alright if you only need to put a form on one page. The form size can be adjusted if you know where to look, but it will reset all forms on your site. If each form could be size adjusted, it would be ideal.

This is a really good module, and is very easy to use. I wish it offered some additional user controls such as radio buttons and check boxes, but otherwise I think it gets the job done.

Without question one of the easiest modules I have ever worked with. Blown away with how quick and great this mod was 5 minutes after I bought it. Highly recommend!

EASY FORM is a breeze to use and even easier to set up. After trying other form modules from different developers that were incredibly difficult to implement and at double the price or more EASY FORM does exactly what I need and I had it running it 5 minutes. I had one small issue that was entirely my fault and the developer resolved the issue that same day. I give this module a 5 star rating!!
