I add the module, but the the "Apply Online " is not function?

I LOVED working with these guys to make the customizations that we needed, and just can't say enough about how professional they are, how easy they are to work with, and how great the end result was. FANTASTIC and HIGHLY recommended, if not just for the module, for the chance to work with true professionals!

Excellent module with fantastic support!
I have been using this module for three years on http://library.loudoun.gov/Default.aspx?tabid=344 and several other locations throughout the site and it makes form generation effortless. It also supports the ability for the end user to upload files and to email.
I had a recent problem with one of the reports the module generates throwing an error message. The developer was quick and very persistent in getting my issue solved.
I believe when I first purchased the module it was developed by a different group/individual because I needed the license released about 2 years ago and there was a long delay in the response. That is not the case anymore every question was answered inside of 24 hours and they were even working on a resolution to my recent issue on Christmas morning.
Highly recommended!

This module has terrific potential...if it would actually work. I have spent countless hours troubleshooting and trying to execute workarounds so I could use this module on just two different sites. The support team is very defensive and usually blame something on the site itself although they are responsive. They gave me an upgrade to my purchase however that just seemed to feed the fire. I will update this review if I find that the problems go away but for right now, I don't think this module is ready for primetime unless you're a programmer yourself.

Thanks Greg for your review. However, this module has been sold for hundreds of clients for its innovation. The 2.5 version sales are growing faster than old versions with no such complaints. We offered you refunds, and our developers already sent you suggestions and possible causes for the problems at your side to help you as much as we can. If those issues were real problems in our module, we would be more than happy to fix them and avoid their occurrence with other and future clients. Thanks again.
Good module I can't say the opposite but license was not clear at all. I NEVER BUY single website license, it's in opposition to community and other modules offer the same without this restriction in very reasonable price. I would rather pay much more and use it as I wan then to pay per use and depend on the reply each time you need it.

Thanks for your review. We started to offer Enterprise editions for all our modules. Enterprise edition allows unlimited installation to unlimited websites. Auto Anchor and Content Accordion is on roadmap.

I'm using this as a very basic drop-down vertical menu and it works great, is easily configurable and fully compatible with my DDN site. It's low price and worth every penny. Now, if the developer would only give the user the option of coloring the menu (or better still using a gradient background to blend in better with site themes) it would be perfect!

I submitted a question detailing a bug that I found in the module days ago and absolutely no response. It doesn't appear they are interested in customer support. Very disappointing. I would not purchase this product again.

I found this module to be very easy to use. There are a few features that would make it even better. One being a upload button when adding a image to the form.
If you are looking for a simple tool to create a custom form, I would recommend this module.
This group also offers a survey form with additional features that I believe should be included in this package as well.

Quite a nice product, and works as advertised.
However, customer service for pre-purchase customers is less than stellar. It took 4 emails just to communicate my issue (trial option wouldn't work), and it was never resolved.
After a week of waiting for a resolution, I ended up buying it before I could try it.

Poor support. Module stopped working after 8 months of purchase and I was told that they are no longer providing support for this version that I need to update to the newer more expensive version. What a waste of $120. Module has lots of bugs and issues.

FINALLY...a form builder that works as it is supposed to, AND is easy to use! This is now my new favorite module for DNN! It is well thought out and extremely easy to use, and functions flawlessly! Although not as dynamic as some may need, it certainly will meet the needs of 90% of DNN users. From what I hear, the future versions will be even better...if that's even possible! GREAT MODULE!!

I would really recommend this module if you're looking for something nice and simple, easy to use and most of all - quick to set up. Love the drag and drop feature - would love to see more modules like this.

Limited layouts and customization. Not capable of handling multi page forms or long forms (beyond 20 or so fields gets slow and buggy). Great for quick and clean form handling. Wish you could re-send lost form data. Also I wish someone would makea module to allow you to customize item payments to charge a convenience fee for paypal transactions. I use this for rent collection and user needs to input any amount they want to pay and I want to increase that payment a fee to get a net result to my account. Overall though - good value and reliable.

I was able to create a form and set it up for collecting payments through Authorize.net in minutes just like the description says. Customer service has been outstanding as they helped me through some self inflicted license issues.
Suggested improvements: Allow for DotNetNuke tokens as default input values. Allow for quantities to be entered by the products purchased in the payment section.

I have been wanting to do have a simple summary of our upcoming events. This is a great little module and is perfect for any home page or events page. I also received awesome service when I needed it. I highly recommend it!!!
