This powerful yet simple module allows you to configure a form with anywhere between 1 to 200 lines, each one containing a field or label. You may name each field as you please. You may select from the usual range of field types, including text box, text area, check box, radio button, etc.. You may specify if the field is mandatory.
You may also configure sections with headings using the Heading 1 through Heading 4 (uses through styles) to separate your fields into logical groups. You may also add simple paragraphs or lines of text or questions using the label field (normal text).
Do you have a special need? The HTML field type allows the use of custom html code for media, links, images or other formatting, making your form visually appealing and easy for your user to navigate.
This flexible custom form builder module is great as a simple Contact Us form and just as handy when it comes to more comprehensive multi-section registration forms, surveys, questionaires and many other forms for online data collection applications. You can even string a series of forms together to take the user through a series of steps.

The key aspects of the module's Field Settings are as follows:
- Number of Fields: This is where you first set the number of fields you would like to display in your form. Use the refresh button to display them below. You may also add and delete individual rows as required.
- Label Width: Set the percent of the container to be used for the column containing the field names. The remainder will be used for the data entry portion of the fields.
- Type: Each line may contain a certain type of field, label or heading. You may select from a variety of field types (see below)
- Name: What you want the field label to display. Note: Any field with a name will be displayed. To add a blank line you may prefer to use a label or html.
- Mandatory: Use if valid data must be entered before the form is submitted. The field will automatically display a mandatory mark (*)
- Lines: Some field types (eg Text Area) enable you to specify the height, in lines, of the field's data entry section.
- Width: The width of the data entry section of each field may be specified. For example, it is common for email address fields to be wider than phone number fields. Width specification is also useful for managing the number of columns displayed for check box and radio button options. 100% will display them in 1 column, 50% in 2 columns, etc.
- Options: Where a field type (eg combo box) has a number of options for the user, please configure these in a comma separated list form.
Some more details on the available field types:
- Check Box - multi-select, comma separated option values
- Combo Box - comma separated list of option values, defaults to first value in list.
- Email - email address expected and validated
- Email Receipt - Validated email address that will receive the same email sent to the “Email To” addresses.
- Headings - h1 to h4 section headings using your existing CSS styles
- HTML - special field for custom formatting, links, media, inserts (not sent to email recipients)
- Label - normal style full width text
- Radio Buttons - only one can be chosen, you set the options
- Text Area - a larger multi-line text box
- Text Box - for users to enter text, numbers, dates, etc.

Email field data is validated and mandatory fields must be completed before submission. Your form includes an optional CAPTCHA to minimise potential spam issues. The submitted form may be configured for one or more email recipients. All the submitted data (except the special html field type) is neatly presented in an email which is sent to the specified recipients.

The key aspects of the module's Email Settings are as follows:
- Email To: Email will be sent to this comma separated list of email addresses when the submit button is pressed.
- Email From: This email address will appear as the sent-from email.
- Subject: This text will appear in the subject line of the email.
- SMTP Server: The SMTP server used to send out the email.
- Port: The port used for email - typically 25
- Authenticate: Whether or not authentication is required for the specified SMTP Server
- Username: Authentication User name
- Password: Authentication password
- Requires SSL: Whether or not SSL is required for the specified SMTP Server
The key aspects of the module's General Settings are as follows:
- Turn CAPTCHA on or off
- Define the confirmation message displayed after the user selects the submit button.
- Set up an optional redirect URL to enable a new URL to be oppened after the user selects the submit button.
One more thing! This module may be used on multiple pages of your DNN portal. Each instance may have a totally different configuration :)
