Many issues for this module: 1. Creator does not respond to bug fixes 2. Buggy software, lots of SQL errors 3. Required fields ignored when submitting 4. Reloading form page submits blank data 5. No validations of any kind 6. No Import/export of forms, takes forever to create a complex one 7. Not easy to manage forms or fields. 8. Many more not listed here 9. You get what you pay for ($15)
